چکیده: 

Background: Recently, Bariatric and Metabolic Surgery (MBS) has received much attention for weight loss and treatment of obesity-related comorbidities. Since attitudes about health care are often formed during medical education and the level of knowledge about MBS among general practitioners is still not satisfactory, there is a need to improve general practitioners' awareness. This study aimed to investigate the knowledge and attitude of medical internship students of Islamic Azad University, Mashhad Medical Sciences Unit, towards MBS in 2022. Method: All medical interns of Islamic Azad University, Mashhad Medical Sciences Unit, were included in this cross-sectional study. A researcher-made questionnaire including 15 questions, was used. This questionnaire was first translated into Persian. To check the content validity and reliability of the questionnaire, a working group consisting of seven academic surgeons was formed, and the items were reviewed and modified, and after confirming the reliability and validity the questionnaires were provided to the students using Google Forms, and the data were analyzed through SPSS version 16. Results: 105 participants with an average age of 26. 49 ± 8. 8 years were included, of which 31 were men and 74 were women. The level of knowledge about MBS in interns was favorable. The attitude of interns towards MBS according to gender and body mass index and passing the surgical course did not show any significant difference (p-value>0. 05). Conclusion: This research showed that medical interns' knowledge and attitude regarding the characteristics and complications of MBS are generally favorable.

Introduction: Gender insensitivity (lack of gender awareness) in the physician’ s professional role and practice can lead to outcomes such as gender discrimination and gender-based harassment in various areas, such as medical education, career opportunities, and specialty selection. The purpose of this study was to reveal the place that the concept of gender occupies in medical education in Turkey by canvassing the opinions of final-year medical students regarding theories of gender roles and socialization, academic capitalism, and liberal feminism. Methods: This study was a Cross-sectional survey. The study population consisted of 1739 interns in six medical faculties in four different geographical regions of Turkey. The reason behind the selection is having different socio-economic factors. Students were selected by simple random sampling technique. For determining it is jumped five students from the lists in faculties. For the validity and reliability of the 14 survey questions, 5 expert opinions were examined and the preliminary instrument was applied to 10 students. Chi-square test was used for comparative analysis. Results: The students who stated that their gender had not affected their educational lives during clinical training reported that it had adversely impacted their internships (P<0. 001). More male students than female ones stated that male physicians were more confidence-inspiring and more industrious, that they managed better, and that they were more likely to recommend a male surgeon. A high number of students reported being undecided on the subjects of concepts or behaviors concerning gender/gender inequality. Conclusion: We recommend greater focus on role modeling and purposeful teaching of gender concepts from the earliest stages of medical education, with particular concentration on gender culture within a process of change involving all hospital personnel in order to prevent gender discrimination.

مقدمه: چالش های آموزشی مقطع پزشکی عمومی به عنوان مهم ترین مقطع دوره پزشکی اثرات نامطلوبی در حرفه آینده پزشکی دانشجویان این مقطع و متعاقب با آن در سلامت افراد جامعه دارد. بدون تردید شناخت این چالش ها از طریق نظرخواهی از دانشجویان این مقطع می تواند گامی موثر درارتقای کیفیت آموزش خصوصا آموزش بالینی به همراه داشته باشد. . روش کار: پژوهش حاضر یک مطالعه کیفی از نوع تحلیل محتوا بود که در اواخر سال 1398 انجام شد و جامعه مورد مطالعه آن شامل دانشجویان مقطع کاراموزی و کارورزی دانشکده پزشکی مشهد بود. روش نمونه گیری یه صورت هدفمند صورت گرفت. روش گرداوری داده از طریق مصاحبه نیمه ساختاریافته صورت گرفت و مصاحبه ها تا حد اشتباع ادامه یافت و در نهایت تعداد 18 نفر از کاراموزان و کارورزان به عنوان نمونه انتخاب شدند. داده ها با استفاده از نرم افزار تحلیل و تفسیر گردیدند. نتایج: از تجزیه و تحلیل داده ها چهار مقوله یا درون مایه اصلی و یازده مقوله فرعی استخراج گردید. مقوله های اصلی و زیر مقوله های آن عبارت بودند: از ساختار آموزش(برنامه آموزشی) با موضوعات فرعی عدم تناسب محتوا آموزشی با نیازها، عدم تناسب زمان روتیشن ها. مقوله اصلی فعالیت های یادهی و یادگیری با زیرمقوله های آزمون های غیر استاندارد، ضعف در آموزش مجازی، عدم تطابق آموزش تئوری و عملی، ضعف شیوه آموزش و عدم کاربیت یافته های نظرسنجی. مقوله اصلی تعاملات حرفه ای با زیر مقوله های گاردهای رزیدنتی و عدم احترام متقابل استاد به دانشجو و درنهایت مقوله منابع با زیر مقوله های فضای آموزشی وتسهیلات رفاهی. نتیجه گیری: با توجه به یافته های پژوهش، شناسایی موانع و چالش های پزشکی عمومی و برنامه ریزی جهت مرتفع ساختن آن، نیاز به یک رویکرد اقتضایی و سیستمی در حوزه آموزش دارد.

سابقه و هدف: با توجه به پژوهش های پیشین رابطه میان مقابله با شرایط پراسترس و بسیاری از متغیرها از جمله افسردگی، سلامتی، مذهب، حمایت اجتماعی، اشتغال و... صورت پذیرفته است. در این میان، متغیری که به آن کمتر پرداخته شده است معنویت می باشد. بنابراین این مطالعه با هدف بررسی تاثیر رشد معنویت بر راهکار مقابله با شرایط استرس زا انجام شد.روش بررسی: در این مطالعه توصیفی همبستگی، 384 نفر از دانشجویان دانشگاه آزاد اسلامی واحد پزشکی تهران به وسیله روش نمونه گیری در دسترس انتخاب شدند. ابزار مورد استفاده شامل آزمون مقابله با شرایط پراسترس CISS که به منظور ارزیابی سه سبک مقابله یعنی مقابله مساله مدار، مقابله هیجان مدار و مقابله اجتنابی تشکیل شده و آزمون سنجش رشد معنوی SAI به منظور ارزیابی 6 زیرمقیاس آگاهی از وجود خدا، پذیرش واقعی، ناامیدی، بزرگنمایی، مدیریت برداشت و بی ثباتی بود.یافته ها: بین رشد معنویت و مقابله مساله مدار ارتباط معنی دار و مستقیم وجود داشت، این بدین معنی است که با افزایش نمره «رشد معنویت»، «میزان مقابله مساله مدار با شرایط پراسترس» در آن ها افزایش می یابد.نتیجه گیری: این پژوهش نشان داد ارتباط بین رشد معنویت و سه بعد شاخص های مقابله با شرایط استرس زا معنی دار است. این بدین معنی است که رشد معنویت یکی از پیش بینی کننده های مهم روش مقابله با شرایط پراسترس دانشجویان محسوب می شود.

Background: The imbalance between the public needs and the number of the medical graduates has created problems in recruitment of young physicians who are trained to provide medical and health services in the country.Purpose: To assess the attitudes of interns at Qazvin University of Medical Sciences towards their future profession in 2001-2002.Methods This descriptive study assesses the viewpoints of all 120 interns through a self administered questionnaire.Results: The findings indicated that 77.5% of the students were in the age range of 22 to 27 with the average of 26.4 years. Of all cases 67.5% wished to continue their training to become a specialist and 6.7% decided to work as a general practitioner. Of all students, 95.8% expected their future income to be low, to an extent which will not provide a comfortable life for them. Of all cases, 89.2% believed that the present system of medical education does not give them the necessary capabilities and skills to practice efficiently in the future.Conclusion: The students' viewpoints on fulfillment of their economic needs and provision of a comfortable life for them and their families were negative; however, regarding fulfillment of their mental and emotional needs and participation in a scientific field, they were positive.

Background and Purpose: "Office-based" course in General Medical Curriculum of Shahid Beheshti School of medicine (the reform program) is a four-month course that is presented in the last six months of the program for interns. Office is a major practice setting after graduation for general practitioner in Iran and for this purpose physicians' offices in health care centers have been selected.Purpose of the course is to prepare interns to work independently and they have passed all their courses and had adequate knowledge to practice but they had not enough experience of office-based practice in the community and outpatient settings. This study was designed and implemented aimed to determine the program’s weaknesses and strengths in the range of the course stakeholders ‘questions (5 major questions), and provide proposed solutions to policy-makers in order to improve and promote the program of "office- based education internship" of Shahid Beheshti School of Medicine.Methods: The assessment was conducted in 3 descriptive study and the population under study included 44 interns and 36 physician mentor in 36 health care centers which were all centers under office based education plan in Shahid Beheshti medical school from October till December of 2014.The instruments used for data collection were questionnaires (mentor physician and intern students) and a check list made by the researcher.Mentor physician questionnaire contained 23 items, student questionnaire contained 20 items and documentations review check list and evidence included contained 30 items. Twenty items were on a 4-point Likert-type (weak, less than expected, as expected as and higher than expected), 14 items on a 3-point Likert-type (none, partially, totally), 33 items on two-choice question (Yes/No) and 6 as an open question. All items had the same value. Face and content validity were checked by Scientific Committee and evaluations of Shahid Beheshti University of Medical Sciences.For checking educational achievement, a student self-assessment and mentor assessment were performed in 3status, at the beginning of the course, after 2 months and after 4 months.Results: The gap between design and implementation was at least 14.1%. More than 75% of mentors had desirable conditions in terms of scientific, educational and professional competencies from the perspective of students but mentor physicians' abilities in basic skills of practice was lower compared with educational and professional abilities.Students’ skills at the beginning of the first two months of office-based course internship was lower than expected in all areas and at the end of the 4-month course learning reached 95% to 100%.Per capita, variety and number of patients referring to teach students (90.6% of the centers) were sufficient.66.7% of mentor physicians were interested in working as a general physician and 42.7% were interested in teaching students.52.8% of mentor physicians did not receive any fee and 100% of the recipients were not satisfied with the amount of the fee. In 81.2% of health care centers, work and training space was appropriate and there were some difficulties about equipment.Conclusions: The results of this study showed that health centers are appropriate educational setting for general medical students' office-based course and effective on promoting their essential skills of practice. Although educational and professional competencies of about 75% of mentors was desirable but the necessity of considering mentors' training, becoming more familiar with the goals and standards of the program, as well as the promotion of knowledge at the same time with considering financial motives and job promotion can play an important role in presenting this program.The need to pay attention to provide basic facilities in the centers, as well as continuous and periodic evaluation are other recommendations of the study.

چکیده: 

این مطالعه با هدف ارزیابی مهارت های ارتباطی همه 110 نفر کارورزان دانشکده پزشکی دانشگاه علوم پزشکی قزوین با بیماران در سال 1383 انجام شد. به طور همزمان 110 بیمار نیز به طور تصادفی از همین بیمارستان ها انتخاب شدند. 38.18% کارورزان همواره با بیمار سلام و احوال پرسی و تنها 1.82% خود را به بیمار معرفی می کردند. 21.82% همواره از خود حساسیت نشان می دادند و 10.91% همواره بیمار را از ابهام خارج می کردند. 23.64% اجازه پرسیدن هر سوالی را به بیمار می دادند. از نظر بیماران 49.09% در مورد مشکل خود تفهیم می شدند، 52.73% می توانستند همه سوال های خود را بپرسند و 61.82% کارورزان از خود حساسیت و دلسوزی نشان می دادند. بنابراین پیشنهاد می شود که آموزش ارتباط مناسب با بیماران در برنامه آموزشی کارورزان گنجانده شود.
